Addiction and Mental Health Worker, System Navigator

As a member of the Supportive Housing Team, you will support individuals who may experience addictions and/or mental health concerns and have struggled with housing stability. You will be the first point of contact for referrals into Supportive Housing programs that are open to general referrals from both internal and external sources. You will also provide brief support when occasions arise to those clients that live in our Supportive Housing units. 

The Addiction and Mental Health Worker will also provide information, as requested, about CMHA services and other community services to participants, family members, and professionals.

  Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 3 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

WHAT WE'RE OFFERING:

• What you'll be making -  Starting rate for this role is $27.14per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($30.01per hour for Level 1 – Direct Service, Regulated):
  • Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
  • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated)
  • For internal applicants, grid level placement will occur as per the Collective Agreement
• When you'll work – 70 hours over a bi-weekly period, Monday to Friday 9:30 am to 5:00 pm

• Your pension plan - Immediately eligible for optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP)

HOW YOU WILL BE MAKING A DIFFERENCE:

• Provide and facilitate referrals to mental health and addiction resources as appropriate.
• Liaise with Managers and Coordinators of agency programs to ensure a timely and responsive referral process for those seeking services.
• Advocate, refer and connect clients with appropriate services, supports and resources based on their needs. 
• Advocate on behalf of clients (eg. within hospital and community) to ensure the client's rights are not compromised.
• Assist with short-term crisis management by providing an urgent/emergent response to individuals in need of case management supports
• In collaboration with the client, conduct a variety of assessments (skills, functional ability, needs, barriers, etc.) as appropriate, using a culturally competent approach
• Provide health and wellness support including participation in and/or coordination of Community Care Plans, relapse prevention and planning, wellness/recovery planning, medication education, symptom and medication monitoring, referrals to specialized health services and support with physical health concerns
• Assess the adequacy of resources/supports currently available to the client
• Determine the impact of psychiatric symptoms on functioning
• Identify risk factors related to client and staff safety as well as relapse
• Assess barriers to successful community living (e.g. treatment options, transportation, finances, etc.)
• Assist client to identify their personal goals and preferences and utilize standardized tool 
• Assist client to navigate systems and connect to appropriate resources as well as to explore options and consequences of different choices
• Help individuals work towards their vision of a positive personal future and identify practical strategies for achieving their vision
• Work in partnership with client in all aspects of planning
• On a short-term basis, meet with client regularly to work on specific goals until other permanent supports are in place
• Encourage and facilitate personal growth and development towards rehabilitation and recovery
• Provide needed information and education about mental illness, addiction, coping skills, services, resources, self-help and consumer initiatives/supports
• Identify, develop and maintain good links with a wide range of community resources; continually build positive working relationships with community partners
• Regularly solicit feedback from the individual regarding their needs and preferences
• Regularly seek input from other relevant stakeholders, internal and external, regarding individual progress and incorporate that information into planning and interventions

WHAT YOU'LL NEED TO APPLY:

We require the following qualifications:

• Diploma or degree in a health, social service or a related discipline
• Excellent knowledge of community resources in London-Middlesex
• Basic computer skills
• Valid driver’s license and access to a vehicle to carry out job duties
• Adequate vehicle insurance to carry out job duties
• Ability to attend work regularly

  We prefer the following qualifications:

• Knowledge of a range of crisis prevention and intervention approaches including: crisis prevention strategies, crisis planning, crisis intervention, risk assessment, relapse prevention strategies, crisis services and hospital services
• Knowledge of serious mental illness and/or addiction and its impact on individuals
• Experience working with individuals with a serious mental illness and/or addiction
• Current First Aid & CPR
• Bilingual both official languages, preferred; préférence sera accordée aux candidats qui sont compétents dans les deux langues officielles du Canada
