The Canadian Mental Health Association, Halton Region Branch is seeking a Peer Service Navigator- FAC on a contract for a pilot basis for the period of November 15 , 2025 to April 30, 2027 . The position is a 30 hour (4 days per week) per week role based in Burlington, primarily operating out of Joseph Brant Hospital. The position resides within an integrated team who work closely together to connect and support family members and caregivers of individuals struggling with mental health and/or addictions. Through supporting those who care and support these individuals, we help to improve the health and wellbeing of caregivers in Halton Region. By integrating this service within our client-focused programs and services, we can provide comprehensive wrap-around care for both clients and those that care for them.

An emphasis will be placed on supporting access to step-down services post-discharge from the hospital system with the provision of service navigation, short-term support and resiliency building through education and group trainings/workshops. Please note that the hours for this positon are flexible and may include evenings and/or weekends as required to meet caregiver and family member needs and availability.

The Successful Candidate will:

Ensure appropriate service navigation/coordination and peer support to family members and caregivers with emerging or ongoing mental health needs as part of an interdisciplinary team. They will act as an onsite resource for Joseph Brant staff and other community partners supporting families and caregivers who may be experiencing their own mental health challenges or caregiver fatigue. Service is provided in both 1:1 and group formats.

Responsibilities:

• Develop strategies to assist family members and caregivers to prevent the need for future emergency room visits for concerns that can be addressed with informal or community supports.
• Offer programs and workshops to educate and equip caregivers with tools and strategies to successfully care for family members struggling with mental health and/or addictions.
• Meet with Joseph Brant staff/partners and follow up on any referrals made after hours, on weekends.
• Work with caregivers individually as needed to provide service coordination.
• Focus on strategies for families and caregivers to help support their loved one and care for themselves. Proactively engage with family members/caregivers to create care plans that prioritize access to appropriate supports and resources.
• Run Family Matters sessions for caregivers and families in addition to drop-in sessions weekly.
• Make referrals to a wide variety of community programs and services in Halton region for family members and caregivers.
• Build effective working relationships and maintain contact with representatives of other sectors/organizations in the community.
• Commit to supporting health, safety and wellness according to relevant legislation and agency policies and procedures.
• Complete all required documentation, evaluation tools, and data collection.
• Other duties as assigned.

Qualifications:

• Prior peer experience is required
• At least three (3) years’ experience working with individuals with mental health and addictions concerns and families/caregivers
• Prior experience working within an acute care setting is an asset
• Knowledge of relevant mental health legislation including the Mental Health Act
• Strong communication, critical thinking and interpersonal skills
• Familiarity with community services and resources
• Commitment to the Recovery philosophy of service provision
• Initiative, creativity, flexibility and an openness to change
• Demonstrated ability to work independently and as part of a team
• Excellent oral and written communication skills
• Crisis intervention and de-escalation skills
• First Aid, CPR and ASIST certifications are assets
• Knowledge of EMHware database an asset
• Proficiency in MS Word and Excel
• Ability to speak a second language would be considered an asset
• The successful candidate must pass a vulnerable screening check
