The Canadian Mental Health Association, Halton Region Branch is seeking a Peer Service Navigator- FAC on a contract for a pilot basis for the period of November 15 , 2025 to April 30, 2027 . The position is a 30 hour (4 days per week) per week role based in Burlington, primarily operating out of Joseph Brant Hospital. The position resides within an integrated team who work closely together to connect and support family members and caregivers of individuals struggling with mental health and/or addictions. Through supporting those who care and support these individuals, we help to improve the health and wellbeing of caregivers in Halton Region. By integrating this service within our client-focused programs and services, we can provide comprehensive wrap-around care for both clients and those that care for them.
An emphasis will be placed on supporting access to step-down services post-discharge from the hospital system with the provision of service navigation, short-term support and resiliency building through education and group trainings/workshops. Please note that the hours for this positon are flexible and may include evenings and/or weekends as required to meet caregiver and family member needs and availability.
The Successful Candidate will:
Ensure appropriate service navigation/coordination and peer support to family members and caregivers with emerging or ongoing mental health needs as part of an interdisciplinary team. They will act as an onsite resource for Joseph Brant staff and other community partners supporting families and caregivers who may be experiencing their own mental health challenges or caregiver fatigue. Service is provided in both 1:1 and group formats.
